i think a couple templates would be good, but should be based on a customizable system as you suggested, allowing the clan officers/owners to specify new textfields or dropdown boxes.
also i plan to extend aliases to include game-specific stats. to use wow as an example, a "wow-enabled" alias would include class, level, stats on resistances, defense, whatever; and when apply to a guild these stats will show up in a nice way to the officers reading the apps.
